Job Description:

Position Overview:
Reporting to the Patient Care Manager of CP2 Hemodialysis, the Registered Practical Nurse (RPN) practices independently within their scope of practice when the patient(s) condition indicates predictable outcome. The RPN delivers nursing care based on a pre-set plan of care with a delineated range of intervention. Collaboration and direction is required from the RN in situations that are beyond the scope of complexity designated to the RPN. The delivery care model is Team Nursing with a staffing mix of both RN’s and RPN’s. In addition, the RPN’s perform a variety of duties to assist the team, in accordance with the scope of practice and standards set by the hospital, Regulated Health Professionals Act and the College of Nurses of Ontario. In this role, you will provide high quality nursing care to patients and their families, based on the philosophy and conceptual framework of Nursing Practice at Scarborough Health Network (SHN).

This position involves rotating between CP2, 2 Central, 4E hemo and the satellite dialysis sites.

Requirements:
• Current registration with the College of Nurses of Ontario
• Current BCLS
• Completion of a medication course for RPN’s
• Nephrology 1 and 2 courses completed from a recognized nephrology nursing certificate program or equivalent combination of education, training and experience (Successful applicants not completed the Nephrology course will be expected to enroll within one year)
• Minimum of one year of hemodialysis experience or equivalent combination of education, training and experience
• Demonstrated assessment skills within the RPN scope of practice
• Excellent interpersonal and communication skills both written and verbal
• Demonstrates excellent problem-solving, decision making and critical thinking skills
• Demonstrates excellent organizational and time-management skills
• Demonstrates excellent team building skills
• Demonstrates initiative and the ability to function both independently and as a multidisciplinary team member in a fast paced demanding environment
• Demonstrates values consistent with SHN's Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation
• Utilize protective equipment provided and identify and reports hazards
• Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
•Strong work ethic and commitment is required
•Demonstrated high standards of work outputs
•Highly punctual
•Demonstrated professionalism and reliability


Asset:
• Patient Assessment course
• Graduate of an RPN diploma program
• Completed Nephrology Nursing program or willingness to complete within 1-2 years
• Experience with renal patient
• Ability to speak another language
