Duties include:
- Develops, coordinates, and facilitates programs that meet the emotional, psychological, physical, and social needs of participants in the Day Program, including hands-on delivery.
- In consultation with Day Program Senior Supervisor and Supports Facilitator Team: schedules, facilitates, and monitors PCP planning for all clients on attendance list. This will include maintaining and updating My Compass profiles.
- Responsible for ensuring the Day Program equipment is maintained and supply materials are purchased and in stock
- Facilitates staff training to Day Program staff as required.
- Initiates and coordinates opportunities that foster participation, including monthly educational presentations from community members.
- Coordinate all Person-Centred Planning meetings, reviews with participants when appropriate.
- Develop and implement intimate care plans and assist in the process when required.
- Ensures any client medication administration complies with SaskAbilities Policies and Procedures. This includes communicating with the pharmacies, as necessary.
Qualifications:
Education:
- Bachelors degree in the Human Service field or equivalent experience
- Valid Class 5 Driver's Licence and access to a working/reliable vehicle
- Valid First Aid/CPR/AED Certification or willingness to obtain within three months
- Proficient computer skills and good knowledge of Microsoft suite of programs / skill in using social media platforms
Experience:
- Two years experience in related field
